AST value: what this liver enzyme says about your liver and muscle

Your AST value shows how much aspartate aminotransferase is circulating in your blood. It is an enzyme that leaks out whenever cells are damaged. AST sits in the liver, but equally in skeletal muscle, heart muscle, kidneys, brain and red blood cells. That is why a raised AST is not, on its own, a liver value. A hard training session the day before explains the result more often than liver disease does. The reference range runs up to 35 u/l in men and 31 u/l in women. What the number means only becomes clear alongside ALT, gamma-GT and CK.

AST (Aspartate Aminotransferase): what this test measures

The test measures how much AST is in your blood, in units per litre (u/l). AST stands for aspartate aminotransferase. Older reports call the same enzyme SGOT.

The AST enzyme does its work inside the cell. There it moves an amino group between two compounds. Your body needs that step to convert amino acids into fuel. As long as a cell is intact, the enzyme stays neatly inside. The moment liver cells are damaged, AST leaks out. It then reaches the bloodstream.

So the result says something about cell damage. But it does not say where that damage sits. AST comes from many tissues at once: from liver cells, from skeletal muscle, from heart muscle, from the kidneys, from brain tissue and from red blood cells. The assay only adds up how much AST is circulating in total.

This is why AST is almost never requested on its own. It usually sits on the report next to ALT, gamma-GT, alkaline phosphatase and bilirubin. ALT is far more concentrated in the liver and is therefore more liver-specific. If your AST is higher than ALT, that often points to muscle rather than liver.

Those four values together tell the story AST cannot tell alone.

A raised AST means cells are breaking down somewhere. It is not a diagnosis. The question is not whether something is wrong, but which tissue is supplying it.

A raised AST has several different causes, and they differ sharply in how often they occur.

Muscular effort comes first. Hard, unaccustomed or eccentric training raises AST for days. In untrained men who did a single heavy weightlifting session, the transaminases climbed to values resembling liver disease. Seven days later they were still elevated. Gamma-GT and bilirubin did not move at all in that same work. That is the tell: AST up, gamma-GT normal and CK up points to muscle.

Heavy alcohol use is second. Alcohol raises AST and shifts its ratio with ALT as well.

Fatty liver disease is third, and the most common reason for a mildly raised result without symptoms.

Medicines follow. Some medicines burden the liver, and certain medicines only do so after weeks. Herbal preparations and supplements belong in the same category.

The blood draw itself closes the list. Red blood cells are full of AST, so damage inside the tube lifts the result without anything being wrong with you.

The magnitude helps you sort them. A mildly raised value, three to five times the upper limit, often means little or fits alcohol or muscle damage. A markedly raised value, more than ten times the upper limit, almost always indicates acute viral hepatitis. Values then stay raised for two to six months.

Creatine kinase is the arbiter. This enzyme comes almost exclusively from muscle tissue. If CK has clearly risen with it, your AST is coming from your muscles.

The De Ritis ratio adds a second layer. In most conditions of the liver, such as hepatitis, ALT sits higher than AST and the ratio stays below 1. A ratio above 2 with a raised gamma-GT is the classic alcohol pattern. Alcohol depletes pyridoxal-5-phosphate, and ALT needs it more than AST does. In fatty liver disease the ratio starts below 1 and creeps up as more scar tissue forms. A ratio rising over years therefore says more than a single measurement.

Note the trap: pure muscle damage also produces a high ratio. That is why gamma-GT and CK always belong beside it.

  1. No hard training beforehand

    Required

    No heavy training, long endurance sessions or anything that leaves you out of breath in the 48 hours before the draw. Normal walking is fine.

  • Water is always fine. Drink as usual beforehand, it makes the draw easier.
  • Take your usual medication, unless your own doctor told you otherwise.

AST (Aspartate Aminotransferase): when is testing worthwhile?

AST is a standard part of a liver panel. Your GP usually requests such a panel for symptoms that could fit the liver, when monitoring medicines that can burden the liver, or as part of a broader health check.

There is one preparation rule that genuinely matters. Do no heavy or unaccustomed training in the 48 to 72 hours before the blood draw. A first CrossFit class, a hard leg day or a half marathon can lift your AST for days. If you did train hard shortly beforehand, the best move is to say so at the draw and repeat the measurement later, rather than being alarmed by a result the body itself explains.

A repeat measurement is often more informative than any single snapshot anyway. Enzyme values fluctuate, and the direction a value is moving in says more than the value itself.

If AST comes back raised, the logical next step is not another AST but the context around it: ALT, gamma-GT and CK. Those three together usually indicate whether this is muscle, bile flow or liver cells. Always discuss an abnormal result with your GP, even when you think you already have an explanation.

Low Levels

A low AST causes no symptoms and carries no clinical meaning. There is no condition that announces itself with an AST that is too low. There is nothing to improve either.

There is one nuance in older people. Part of your AST comes from muscle tissue. Someone with less muscle mass therefore has a lower starting value. As muscle is lost with age, that baseline drops with it.

A result inside the normal range is then slightly less reassuring than the same number in a young, muscular person. The margin to the upper limit has grown. More damage can fit inside it before the value stands out.

That is a reason to read the panel as a whole and to watch the trend over time. It is not a reason to worry about a low number in itself.

High Levels

A raised AST usually causes no symptoms in itself. People do not notice it; they read it on a report. Where symptoms do occur, they come from the underlying cause.

With muscular causes: muscle soreness or stiffness after a hard or unaccustomed session, heavy legs, strength that temporarily disappoints.

With liver causes: fatigue, reduced appetite, nausea, a pressing or painful feeling in the upper right abdomen, and with clear liver problems yellowing of the skin or the whites of the eyes, dark urine and pale stools.

There is one situation that does not tolerate waiting. After extreme exertion, a crush injury, overheating, certain medicines or a severe muscular reaction to a statin, muscle tissue can break down on a massive scale. CK is then often many thousands of u/l, the muscles become markedly painful and swollen, and the urine turns dark and cola-coloured. The kidneys are at risk in this situation. This is a reason to seek urgent medical care, not to order another blood test and wait for the result.

The biggest gain is not in what you change but in when you have blood drawn. Schedule the draw at least 48 to 72 hours after your last heavy or unaccustomed session. If you have just started a new programme or finished a race, waiting a little longer is better. That way you measure your baseline rather than the aftermath of your last session.

Alcohol is the second dial you can turn. Drinking heavily on a regular basis raises AST and shifts its ratio with ALT as well. A period without alcohol often shows clearly, on a repeat measurement, how much of the result was attributable to it.

Weight and movement work more slowly but more structurally. Fatty liver disease is a common reason for mildly raised liver values, and gradual weight loss combined with regular exercise is the best-supported approach. Do not expect an effect within a week.

Go through your medicines and supplements with your GP or pharmacist, including over-the-counter products and herbal preparations. Never stop a prescribed medicine on your own initiative because of a blood result; discuss it first.

Finally, keep the draw itself calm. A difficult puncture with a lot of "milking" of the arm can rupture red blood cells inside the tube, and that lifts the measured AST artificially.

What is AST in the blood?

AST is an enzyme that sits inside your cells and only reaches the blood when cells are damaged. It occurs in the liver, but also in skeletal muscle, heart muscle, kidneys, brain and red blood cells. The AST value in blood is therefore a measure of cell damage, without the assay itself being able to indicate which tissue the enzyme came from.

What is the normal range for AST?

In adult men the reference range runs up to 35 u/l and in women up to 31 u/l. The difference is mainly because men carry more muscle mass on average. Laboratories use slightly different upper limits from one another, so always read your result alongside the reference range printed on your own lab report.

What is the difference between AST and ALT?

Both are transaminases, but ALT is far more concentrated in liver cells and is therefore more liver-specific. AST additionally sits in muscle, heart, kidney and red blood cells. A raised ALT points more towards the liver, while a raised AST without a raised ALT often comes from muscle tissue. The ratio between the two is more informative than either number alone.

What if AST is too high?

A raised AST calls for context, not panic. Hard training in the preceding days is the most common harmless explanation. Your GP will look at ALT, gamma-GT and CK alongside it to work out whether this is muscle, bile flow or liver cells, and will usually repeat the measurement before any further steps follow.

How quickly do AST and ALT come back down?

After a single heavy effort AST can stay raised for a week or more; in research on untrained men it was still elevated after seven days. When the cause resolves, such as a medicine being stopped, recovery runs over weeks rather than days. A repeat measurement taken at a quiet moment shows whether the value is genuinely falling.

Does a high AST mean a heart attack?

No. AST was indeed used as a cardiac marker decades ago, but troponin has taken that role over entirely because it is far more specific to heart muscle. AST is no longer used today to confirm or exclude a heart attack. With acute chest pain, ordering a blood test is not the route in any case: call the emergency services immediately.

Can a difficult blood draw affect the AST result?

Yes. Red blood cells are full of AST, so if cells rupture inside the tube the measured value rises without anything being wrong in your body. A difficult puncture, a narrow needle or a shaken tube can cause this. If AST, LD and potassium rise together while nothing else fits, a fresh draw is the logical next step.
