Hematocrit: normal values and what they mean

Your hematocrit shows which share of your blood consists of red blood cells. Dutch laboratories report it as a fraction. A result of 0.45 l/l therefore stands for 45 percent.

Hematocrit is almost always measured alongside haemoglobin and the red cell count. It is a concentration measure, so your fluid balance feeds into the result. Being under-hydrated raises the value without a single extra red cell being made.

One isolated hematocrit value therefore says little. The number gains meaning next to your haemoglobin, your MCV and the timing of the draw.

Hematocrit: what this test measures

Hematocrit is the share of your blood volume made up of red blood cells. Blood consists broadly of plasma and cells. Red blood cells account for by far the largest part of the cellular fraction.

A hematocrit of 0.45 l/l therefore means 45 percent of your blood volume is red cells.

The measurement used to be made by spinning blood and reading off the red cell layer. Modern cell counters work differently. They calculate the value from the red cell count multiplied by the mean cell volume, the MCV.

Star-shl lists the method for this test literally as "Berekening", meaning calculation. That explains a property which is often missed. Any measurement artefact that shifts the MCV shifts your hematocrit with it.

Reference values differ per laboratory and per method. Star-shl publishes the values below for hematocrit (code BH01, EDTA blood). The laboratory cites the Handboek medische laboratoriumdiagnostiek as its source.

Children therefore sit in a different band from adults. Newborns run well above that band in the first months. Always use the range printed on your own report.

A useful rule of thumb links hematocrit and haemoglobin. The hematocrit percentage is roughly three times the haemoglobin in g/dl. The Netherlands, however, reports haemoglobin in mmol/l.

Convert first: mmol/l times 1.61 gives g/dl. If the ratio still deviates sharply, that is itself a signal.

Hematocrit: why this value matters

Hematocrit measures a concentration, not an amount. That distinction drives almost every interpretation error made with this value. The result is a ratio between cells and plasma.

If the plasma changes, the result changes, even when the red cell count stays the same.

Dehydration is the clearest example. Drinking too little, a sauna, a heavy sweat session or a diuretic all shrink the plasma volume. The cells then sit closer together and the hematocrit rises.

Not one extra red cell has been made. This is the most common reason for an apparently high result.

The reverse holds too. Fluid overload lowers the value. In pregnancy that happens physiologically: plasma volume rises faster than red cell mass.

Plasma volume also expands in well-trained endurance athletes. Both groups see a lower hematocrit with nothing wrong.

If a raised value persists despite good hydration, other explanations apply. Smoking loads the blood chronically with carbon monoxide and lifts production. Sleep apnoea and chronic lung disease do the same through a lower oxygen level.

Living or training at altitude works along the same route. Testosterone therapy, anabolic steroids and EPO raise the value directly. A primary bone marrow cause such as polycythaemia vera is rare and needs medical assessment.

A lowered value shares its differential diagnosis with a low haemoglobin. Blood loss, a shortage of iron, vitamin B12 or folate, red cell breakdown, chronic disease and reduced kidney function all belong there. Dilution is an additional separate cause.

One timing point matters clinically. Immediately after acute bleeding the hematocrit has not yet fallen. You lose plasma and cells in the same proportion, so the concentration is initially unchanged.

Only once the body shifts fluid into the bloodstream does the value drop. That takes hours. A normal result shortly after bleeding therefore does not exclude blood loss.

Hematocrit: when is testing worthwhile?

Hematocrit is rarely requested on its own. It belongs to the blood count and comes from the same EDTA tube as haemoglobin, the red cells and the MCV. That is also how the result is best read.

Common reasons are complaints that fit anaemia. Think of persistent fatigue, breathlessness on exertion, pallor or dizziness. The opposite picture is a reason too: headache, a flushed face, or itching after a hot shower.

The value is also followed in known conditions. Kidney disease, chronic lung disease and sleep apnoea belong in that group. Under testosterone therapy, periodic monitoring of hematocrit is standard practice.

Preparation determines how usable the result is. Drink normally on the morning of the draw. Avoid a heavy sweat session or sauna in the hours beforehand.

Both raise the value artificially.

The draw itself counts as well. Standing or sitting for a long time beforehand causes haemoconcentration. A tightly applied tourniquet does the same.

The result can come out a few percent higher than in blood drawn lying down. If you repeat the measurement, choose a comparable time and the same conditions.

Hematocrit: symptoms of a high or low value

Low Levels

A low hematocrit means relatively few red cells are available to carry oxygen. The complaints therefore run parallel to those of anaemia. Fatigue that does not lift with rest comes first.

Breathlessness on exertion, dizziness on standing and pale skin also occur. A faster heart rate fits the picture too, because the heart compensates for the reduced transport capacity. Cold hands and feet, headache and loss of concentration are regularly reported.

Severity depends heavily on how quickly the value fell. A gradual decline is often tolerated for a long time without clear complaints. None of these complaints identifies a cause on its own.

High Levels

A high hematocrit makes the blood more viscous. That explains the pattern of complaints that goes with it. Headache and dizziness are reported most often.

A red or flushed face, blurred vision and ringing in the ears also occur. Itching after a hot shower or bath is a striking and fairly specific feature. Fatigue, tingling in the hands or feet and excessive sweating fit the picture as well.

Many people with a mildly raised value notice nothing at all. The result then shows up during a routine test. Complaints say little about how high the value is, and the height says little about the cause.

Hematocrit: lifestyle and this value

The largest gain sits in the conditions around the measurement itself. On the day of the draw, drink what you normally drink. Anyone arriving dehydrated gets a result describing their fluid balance rather than their blood.

Smoking is the most modifiable cause of a structurally raised value. Carbon monoxide occupies part of the haemoglobin, so the body increases production. After quitting, the value falls gradually, because red cells live about 120 days.

Expect months rather than weeks.

Untreated sleep apnoea works along the same route. If loud snoring, breathing pauses and daytime sleepiness sound familiar, discuss it with your doctor. It is one of the most frequently missed causes of a persistently high value.

At the low end, diet mainly helps by supplying building blocks. Iron, vitamin B12 and folate are needed to make red blood cells. A varied diet usually covers that requirement.

Do not start supplements on your own initiative for a low value. Without a demonstrated deficiency it does not help, and iron accumulates in the body.

Finally, exercise improves circulation but does not raise your hematocrit. With endurance training the value even falls slightly through plasma volume expansion. That is a favourable adaptation, not a deficiency.

Can dehydration affect my hematocrit?

Yes, and it often does. Hematocrit is a concentration measure, so less plasma automatically gives a higher result. Drinking too little, a sauna or heavy sweating shortly before the draw raises the value. The red cell count itself does not change. Drink normally on the day of the test.

What is the difference between hematocrit and haemoglobin?

Haemoglobin measures how much oxygen-carrying protein your blood contains. Hematocrit measures how much space the red cells occupy. The two almost always move together. As a rule of thumb the hematocrit percentage sits around three times the haemoglobin in g/dl. A sharp divergence is itself information.

Do I need to fast for a hematocrit test?

No, fasting is not required for this test. Your fluid balance does count, so drinking normally matters more than not eating. If hematocrit is requested together with glucose or cholesterol, the fasting requirement of those tests applies. Follow the instruction on your request form in that case.

What does an abnormal hematocrit actually mean?

On its own, not a great deal. The value becomes usable next to your haemoglobin, your MCV and the red cell count. The timing of the draw and your fluid balance belong with it. A single mildly abnormal result is often a moment in time rather than a condition. Discuss an abnormal result with your doctor.
