Creatinine: Normal Values and What a High Result Means

Creatinine is a waste product that is continuously produced in your muscles. The kidneys filter it out of the blood and remove it in urine. The blood creatinine level is therefore a core number for kidney function. In Dutch it is written both as creatinine and as kreatinine.

Because production depends on your muscle mass, the same figure reads differently per person. A raised creatinine does not automatically point to a kidney problem. This page explains what high and low values can mean. It also covers the everyday things that shift the result temporarily.

Creatinine: what this test measures

Creatinine is a waste product from muscle that your kidneys filter out of the blood. In Dutch reports it appears as both creatinine and kreatinine. The blood creatinine level shows how well that filtering is going. Results are reported in micromoles per litre, written umol/l.

For adult men aged 19 to 50 the band runs from 61 to 113 umol/l. For women in the same age group it runs from 48 to 91 umol/l. The band widens with age, up to 65 to 149 umol/l above eighty. Reference values also differ per laboratory, so your own report is what counts.

A raised creatinine means more of the substance is staying in your blood. That fits reduced filtration, but it equally fits a large muscle mass. A low creatinine level is almost always caused by little muscle. It is rarely a sign of a kidney problem.

The substance itself comes from muscle tissue rather than from the kidney. Creatine and creatine phosphate in muscle break down spontaneously every day. Roughly 1 to 2 percent of the muscle store is converted this way. Production is fairly constant per person, and that is what makes the measurement usable.

From that follows the core of this blood value: muscle mass in, kidney function out. The figure reads as a kidney number only while your muscle mass is average. If it deviates, the whole band shifts with it. That is precisely why separate bands exist for men and women.

The difference between the bands for men and women is almost entirely down to muscle mass. Female and male kidneys do not in themselves filter differently. From your creatinine the laboratory also calculates the eGFR. That estimated filtration rate usually says more about kidney function than the raw figure.

Creatinine: why this value matters

The kidneys continuously filter waste from your blood and keep your internal balance stable. As long as that goes well, you notice nothing at all. Loss of filtering capacity is gradual and symptom-free in most people. A blood value is therefore often the first thing to go on.

Creatinine is a blunt instrument for that early stage. The value only rises clearly once a substantial part of filtering capacity is gone. That is why a doctor never reads the figure on its own. The eGFR and the protein or albumin level in urine belong beside it.

The best known pitfall is the heavily muscled man. He produces more creatinine, so his value sits higher. His kidneys clear the substance perfectly normally. A result above the band often means nothing at all in his case.

The reverse situation matters more clinically and is missed more often. With little muscle mass the body produces little creatinine. Think of old age, long illness, bed rest or an amputation. A perfectly normal value can then hide clearly reduced filtration.

Cystatin C helps when those two pictures are hard to separate. Almost every cell produces it, and it does not depend on muscle. If creatinine and cystatin C diverge sharply, muscle mass is usually the explanation. Your doctor decides whether that extra test is useful.

Creatine as a supplement raises the creatinine level without loading the kidneys. Simply more raw material enters the muscle, so more breakdown product enters the blood. Research in healthy users shows no fall in measured filtration. The eGFR formula does translate that rise into a lower kidney function.

Some medicines lift the value without touching the kidney itself. Trimethoprim and cimetidine block the secretion of creatinine in the kidney tubules. Serum then rises while true filtration stays the same. Never stop a prescribed medicine yourself, but discuss such a rise with your doctor.

A low value calls for a completely different explanation than a high one. It rarely points to the kidneys and usually to little muscle mass. Pregnancy lowers the value physiologically, because filtration increases considerably. Advanced liver disease lowers it too, since the liver makes less creatine.

Creatinine: how to prepare

  1. Better no hard training beforehand

    Hard exercise in the 48 hours before can raise this value temporarily. Plan the draw on a rest day if you can.

  • Water is always fine. Drink as usual beforehand, it makes the draw easier.
  • Take your usual medication, unless your own doctor told you otherwise.

Creatinine: when is testing worthwhile?

Creatinine is part of any general kidney function check. Your GP usually requests it together with urea and eGFR. A urine test for albumin is often added. That combination describes the kidney more fully than one figure.

Some situations make periodic checking usual. Examples are high blood pressure, diabetes, cardiovascular disease and kidney disease in the family. Long-term use of certain medicines is another reason. Your doctor decides how often that is useful.

Sport and supplements are a second reason to test. Anyone using creatine or training hard wants to know their baseline. A measurement in a quiet week gives the most usable reference. Repeat it later under the same conditions.

Preparation matters more here than for many other blood values. Skip a large meat meal in the hours before the draw. Avoid heavy strength training or a long endurance effort in the two preceding days. Drink normally, so you do not arrive dehydrated.

Always mention it if you use creatine. That single detail changes the interpretation of your result completely. Repeat an unexpected value rather than drawing conclusions from it straight away. One isolated figure is almost never enough.

Creatinine: symptoms of a high or low value

Low Levels

A low creatinine level almost never causes complaints by itself. The value follows your muscle mass, and that changes slowly. What people notice belongs to the underlying cause. The figure is at most a reflection of it.

With declining muscle mass it is about loss of strength and a weaker grip. Clothes start to fit loosely while weight seems unchanged. Rising from a chair or climbing stairs takes more effort. With long bed rest or illness that happens faster.

A low value in pregnancy is normal and reflects higher filtration. On a strictly plant-based diet the value tends to run somewhat lower. In advanced liver disease other complaints matter more than the creatinine itself. A low result without complaints is usually no cause for concern.

If you become weaker or lose weight without a clear reason, mention it. Not because the creatinine is low, but because muscle loss itself deserves attention.

High Levels

A raised creatinine usually causes no symptoms in itself. That is exactly why the value is measured. Signals only appear once filtration is substantially reduced. They are also vague and fit many other causes.

Reported complaints include tiredness, itching, nausea, reduced appetite and poor sleep. Fluid retention around the ankles and eyes also occurs. Some people notice foaming urine or passing less water than usual. High blood pressure and breathlessness can belong to the picture.

None of these complaints proves anything about your kidneys. They occur just as readily in dozens of other conditions. If they persist, discuss them with your GP. Never judge them on this figure alone.

What a raised creatinine is not also matters. It is not a diagnosis and not a measure of how ill someone is. In a muscular body the figure can sit above the band for years. Your doctor weighs it against your eGFR, your urine and your history.

Creatinine: lifestyle and this value

A few things around the blood draw genuinely matter. Skip a large portion of meat in the hours beforehand. Cooked meat contains ready-made creatinine, which enters your blood directly. Do not schedule the draw straight after a hard training session.

Two days of rest before the draw gives the cleanest picture. Make sure you have also been drinking normally. Dehydration concentrates the blood and lifts the value temporarily. An ordinary day gives a result you can compare later.

If you use creatine, always say so. Before a kidney work-up it can make sense to pause it temporarily. Discuss that with your doctor and do not decide it yourself. Ask whether cystatin C would be a useful addition.

For the kidneys in the longer term the familiar basics count most. Well-controlled blood pressure and blood sugar protect the small vessels in the kidney. Moderating salt helps blood pressure. Regular exercise and not smoking belong in the same list.

Long-term high doses of anti-inflammatory painkillers such as ibuprofen burden the kidneys. Discuss prolonged use of these with your GP or pharmacist. Drink enough, but there is no proven extra benefit in overdoing it. No diet repairs a damaged kidney.
